b. insurance card, policy or invoice statement
c. physical vin plate on the vehicle
2. Full name and address of owner(s).
3. Maryland driver's license number and/or date of birth of owner(s).

4. Norman's Limited Power of Attorney, signed by all owners:
A. Original signatures are required.
B. A facsimile or photostat copy is not acceptable.

A. Copy of the trust agreement, certified by a notary as a true and correct copy.
4. Owner of a company:
A. Signature of owner or partner and their capacity to the company.
5. Officer of a corporation:
A. Signature of an officer and his/her capacity to the corporation.
